Prepare for your clinic visit

Choose what is bothering you, tick what applies, and save a one-page card as a picture. Send it to yourself on WhatsApp and show it to your doctor, so you remember everything in a short visit.

Nothing you type is saved or sent anywhere. It stays on this screen and disappears when you close the page.

• Your medicine list, including creams and supplements
• Any allergies, written down
• Previous test results, scans and discharge papers
• Your ID and insurance details
• A relative or friend if you would like support

6. Tick the questions you want to ask

• What do you think is causing this?
• Do I need any tests, and what will they show?
• Do I need surgery, and if so, how soon?
• What are my options if I do not have surgery?
• What are the risks and what is recovery like?
• What warning signs mean I should come back or go to the emergency room?

On a phone this opens your share menu, so you can send the picture to yourself on WhatsApp and show it at the clinic. On a computer it downloads the picture.
